29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       a) Roof replacement work was being done by employee(s) at a residential property at 81 1/2 Pulaski Avenue in Carteret NJ, with no means of fall protection was in use to protect employee(s) on the roof from the hazard of falling 22 feet to the ground below.      Condition(s) were observed to exist on or about 9/8/2015.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) Employee(s) on the roofing crew working at 81 1/2 Pulaski Ave in Carteret NJ used a Werner aluminum extension ladder leaned against the south edge of the home for roof access & egress.  The side rails of the ladder did not extend at least 3 feet above the roof edge; exposing employee(s) to a fall hazard of 22 feet.    Condition(s) were observed to exist on 9/08/2015.
