29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about September 8, 2015, the employer failed to utilize adequate protective equipment (fall protection) for employees accessing materials approximately 15 feet high. An employee rode the forks of a lift truck approximately 15 feet high to move materials off of a rack. The employee was exposed to a serious injury from a fall hazard and was not adequately protected. Appropriate fall protection was not utilized.
