29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    a) Brian's Masonry Restoration and Roofing / 111 West 11th Street, Erie, PA: On or about September 9, 2015 the employer did not have a written hazard communication program. Employees worked with and around hazardous substances that include but is not limited to, gasoline and muriatic acid.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a) Brian's Masonry Restoration and Roofing / 111 West 11th Street, Erie, PA: On or about September 8, 2015 the employer did not certify training conducted with an employee who works from elevated service with the use of fall protection. The work being conducted at this location required the employee to be in an articulating boom while performing work.
