29 CFR 1926.1211(c)(1): (1) Each authorized entrant must use a chest or full body harness, with a retrieval line attached at the center of the entrant's back near shoulder level, above the entrant's head, or at another point which the employer can establish presents a profile small enough for the successful removal of the entrant. Wristlets or anklets may be used in lieu of the chest or full body harness if the employer can demonstrate that the use of a chest or full body harness is infeasible or creates a greater hazard and that the use of wristlets or anklets is the safest and most effective alternative:    a.  On the site at 100 Innovation Court, Delaware, Ohio where an employee entered a permit required confined space to vacuum test the sanitary sewers, the employee was not provided or required to wear a chest or full body harness with a retrieval line attached at the center of the back, thereby exposing the employee to an eight to ten foot fall hazard.

29 CFR 1926.1207(b):  Training required by this section must be provided to each affected employee:    29 CFR 1926.1207(b)(5):  Whenever there is any evidence of a deviation from the permit space entry procedures required by � 1926.1204(c) or there are inadequacies in the employee's knowledge or use of these procedures:    a.  On the site where employees enter a permit required confined space to perform an inspection, the employees were not provided training in the hazards to be encountered when the possibility of oxygen deficient air was encounter and when to determine a retrieval system was needed thereby exposing the employee to a an eight to ten foot fall hazard.
