29 CFR 1926.757(a)(4): Where steel joists at or near columns span more than 60 feet (18.3 m), the joists were not set in tandem with all bridging installed and an alternative method of erection, which provides equivalent stability to the steel joist, was not designed by a qualified person and was not included in the site-specific erection plan. Jobsite: Bays between column lines 4 and 5. Steel joists at or near columns were not hoisted in tandem and were set 'one at a time'. The site specific steel erection plan makes no provision for the alternate erection of steel joists longer than 60 feet.

For many joists, just one row of diagonal erection bridging was installed near the mid-point of the joists. For one joist, no diagonal erection bridging had been installed.

29 CFR 1926.757(e)(4): Bundles of decking were placed on steel joists without all bridging having been installed and anchored and all joist bearing ends attached, and all of the conditions in subparagraphs (e)(4)(i) through (e)(4)(vi) were not met. Jobsite: Bays between column lines 4 and 5. Bundles of decking were placed on steel joists when the joists had not been bridged in accordance with the steel erection drawings. The site specific steel erection plan had no documentation that a qualified person had determined that it was safe to place decking on steel joists without joists being fully bridged and in at least 2 bays decking was placed more than 1 foot of the bearing surface of the joist end.

29 CFR 1926.1417(p): The boom or other parts of the crane were allowed to contact an obstruction. Job site: Girder on column line 5, between columns Q and R. The Tadano GR-750XL crane boom contacted a steel girder and caused it and a supporting column to collapse. As a result, an employee was fatally injured.
