29 CFR 1910.138(a): The employer did not select and require employees to use appropriate hand protection when employees hands were exposed to hazardous condition(s): a) Throughout facility - The employer failed to require the use of hand protection for employees who were cleaning up spills of hazardous chemicals such as, but not limited to, Polycon Manufacturing Sand Mix that contained crystalline silica. Employees were exposed to skin irritations and chemical burns.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: A) Throughout facility - The employer did not establish and implement a written hazard communication program to employees working with chemicals such as, but not limited to, Polycon Manufacturing Sand Mix (crystalline silica), Nalco Coating Defoamer, Citrus Oil Dist (Class III flammable), Sucrose Acetate Isobutyrate, Ink Washable Sketch (Class III flammable), polymix PA-660A, Ene Electropolish (phosphoric acid and sulfuric acid - Corrosive) and propane gas. Employees were exposed to fire hazards, chemical burns to skin and eyes, and respiratory tract irritations. AZ Atlanta was previously cited for a violation of this Occupational Safety and Health Standard, 29 CFR 1910.1200(e)(1), which was contained in OSHA inspection number 314643834, citation number 1, item number 6, and was affirmed as a final order on August 8, 2012, with respect to a workplace located at 690 Airport South Parkway, Atlanta GA 30349.
