29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.  (a) Work site was located at Flex Spaces at Kearny Point, 78 John Miller Way, Kearny, NJ.  An employee was observed working from a mobile scaffold without having that platform security chain secured to prevent him from falling off.  Violation observed on or about 9/2/2015.
