29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: a) Jobsite - On or about September 5, 2015 and at times prior thereto, employees were bolting bar joists to the structural steel beams when an employee fell approximately 20' from the exterior steel beam he was walking/working on. The employee was wearing a harness but was not tied off to an anchor point.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a) Jobsite - On or about September 5, 2015, an employee was working on a steal beam and fell approximately 20' resulting in him being hospitalized. The employer never reported this hospitalization, it was reported by the Wallkill Police Department on September 8, 2015.
