29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about September 9, 2015, the double end borer machine was not guarded at the following pinch point locations: a. The left and right ends where the piston moves inward, stopping the movement of the bore at the location where it engages a microswitch, thus creating a pinch point against the block. b. The center of the machine where the part is placed; As the borer moves in from each end toward the part in the center the guideposts close against the platen creating four pinch points, one on each side in the front of the clamped part and two behind where the part is clamped.
