29 CFR 1926.20(b)(2):Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. Location: Picture window openings on south end of site at 2000 Ronald Reagan Drive, Cincinnati, Ohio a: On or about 8/28/15 a tripping hazards was created because trash storage areas were not kept free of accumulated materials. A subcontractor employee working on the 4th floor tripped on accumulated materials.

29 CFR 1926.502(j)(3): Toeboards shall be a minimum of 3 1/2 inches (9 cm) in vertical height from their top edge to the level of the walking/working surface. They shall have not more than 1/4 inch (0.6 cm) clearance above the walking/working surface. They shall be solid or have openings not over 1 inch (2.5 cm) in greatest dimension. Location: Picture window openings on floors 2, 3, 4 and 5 at 2000 Ronald Reagan Drive, Cincinnati, OH a: On or about 28 August, 2015 toeboards used did not measure at least 3 1/2 inches in height.
