29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports: a) 1639 South 22nd Street, Philadelphia, PA - The employees were working off of a fabricated frame scaffold approximately 42 feet long, 18 feet high and 5 feet wide which was not fully planked or decked on all working levels between the front uprights and the guardrail supports. This hazard exposed the employees to fall hazards. This condition was observed on or about March 10, 2015.

29 CFR 1926.451(e)(1): Where scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers or stairway-type ladders was not used for access: a) 1639 South 22nd St, Philadelphia, PA - The employees were working off of a fabricated frame scaffold approximately 42 feet long, 18 feet high and 5 feet wide which did not have proper access for the employees to reach the various working levels of the scaffold. This hazard exposed the employees to a fall hazard. This condition was observed on or about March 10, 2015.

This hazard exposed the employees to a fall hazard of approximately 18 feet. This condition was observed on March 10, 2015.
