29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems:      a)  Employees performing carpentry and roofing work were exposed to fall hazards of approximately 18 feet.    Bravo Roofing, Incorporated was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 315298919, citation 1, item  1 and was affirmed as a final order on April 28, 2011, with respect to a workplace located at 2606 Dempster St., Park Ridge, IL, 60068.    No abatement certification or documentation is required for this item.

29 CFR 1926.503(b)(1):  The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:      a)  The employer did not maintain records or certification of fall protection training for employees exposed to fall hazards.      In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
