29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a)  On or about 9/9/2015, at the above addressed jobsite, an employee scraping excess cement from the lintel was exposed to a fall hazard of approximately 25 feet while working from a one-tier scaffold located at the 3rd level west end section of building 1.      b)  On or about 9/9/2015, at the above addressed jobsite, an employee laying block on a balcony was exposed to a fall hazard of approximately 25 feet while working from a one-tier scaffold located at the 3rd level east end section of building 1.

29 CFR 1926.502(d)(6)(v): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any object which is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself:    On or about 9/9/2015, at the above addressed jobsite, an employee laying block on the 3rd level at the east section of Building 1 was exposed to a fall hazard due to the improper rigging of the fall prevention system whereas the shock absorbing lanyard's snaphook was attached to safety latch connected to a self-retractable lifeline.
