On or about September 9, 2015, on the south facing part of the building project; the employer did not fully plank the scaffold being used by employees to brick building at heights greater than 10 feet.

On or about September 9, 2015, on the south facing part of the building project; the employer allowed employees to use crossbraces as a means of scaffold access.
