29 CFR 1910.137(b)(2)(viii): Electrical protective equipment was not subjected to periodic electrical tests. Test voltages and the maximum interval between tests shall be in accordance with Table I-5 and Table I-6:     a) On or about September 9. 2015, at the above location workplace employees were required to work with 480 volt electricity and used electrical insulating gloves that were not subjected to periodic electrical tests in accordance with Table I-5 and Table I-6. Employees were thereby exposed to hazards associated with live electrical parts.

The conduit at the motor was cut and not suitable for the damp location. The condition of the conduit was not in accordance with instructions included in the listing or labeling. Employees were thereby exposed to electrical hazards.

The wiring was not enclosed in an approved box or other approved enclosure.
