29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections, to ensure that before employees were required to gain access to machines, including the Bridgeport CNC GX-710, for tool changes, servicing and maintenance, where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine would be isolated, and rendered inoperative. Employees were exposed to hazards likely to cause amputation.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.178(l)(2)(ii): Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.    The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. 29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    On the Bridgeport GX-710- The machine had the interlock device on the movable door guard disabled, allowing employees to gain access to the point of operation during operating cycles. This exposed employees to point of operation and caught in hazards likely to cause amputation.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
