29 CFR 1910.132(a): Protective equipment was not used and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards encountered in a manner capable of causing injury or impairment in the function of any part of the body: a) Electralloy, 175 Main St., Oil City, PA - Ingot Pouring Platforms. On or about September 10, 2015, employees working from the platforms were potentially exposed to burn hazards from molten metal pouring operations. Employees did not wear/utilize aluminized protective clothing designed to prevent thermal burns to the arms, torso, back and, legs. Employees did not wear the aluminized protective equipment while performing activities that included, but were not limited to, removing cardboard covers from tops of molds and pouring molten metal from the ladle. b) Electralloy, 175 Main St., Oil City, PA - Ingot Pouring Platforms. On or about September 10, 2015, employees working from the platforms were potentially exposed to burn hazards from molten metal pouring operations. Employees were not required to wear aluminized gloves and/or equivalent protection designed to prevent thermal burns to the hands. Employees wore standard gloves during the metal pouring operations. Employees did not wear the aluminized protective equipment and/or equivalent protection while performing activities that included, but were not limited to, removing cardboard covers from tops of molds and pouring molten metal from the ladle. c) Electralloy, 175 Main St., Oil City, PA - Ingot Pouring Platforms. On or about September 10, 2015, employees working from the platforms were potentially exposed to burn hazards from molten metal pouring operations. Employees were not required to wear face protection designed to prevent thermal burns during the pouring operations. Employees did not wear the face protection while performing activities that included, but were not limited to, removing cardboard covers from tops of molds and pouring molten metal from the ladle.

29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail and intermediate rail: a) Electralloy, 175 Main St., Oil City, PA - Bottom-Pour Platforms. On or about September 10, 2015, and times prior thereto, the platform decks were equipped with a chain guardrails. The chain guardrail systems were not equipped with a mid-rails (intermediate rail). The condition exposed employees to a fall hazard of at least 12 feet.
