29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:    a)  At the jobsite, along the front porch of the building, on or about 9/8/15:  Employees were using a ladder jack scaffold to access a section of the roof and to transfer roofing material to the roof where; the working platform was made with a section of extension ladder, not designed for this purpose and without the structural integrity in a horizontal placement to ensure the required load requirements, where the scaffold platform was placed so that the left side was in close proximity to supply power lines, and where no fall protection was provided for employees exposed to greater than a 10 foot fall to the ground.       Abatement certification must be submitted for this item.

29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.  a)  At the jobsite, along the front porch of the building, on or about 9/8/15:  Employees were using a ladder jack scaffold to access a section of the roof and to transfer roofing material to the roof where the working platform was made with a section of extension ladder, where a length of plywood was placed over the rungs to provide a walkable surface, and where that plywood did not extend over the entire length of the platform.  Abatement certification must be submitted for this item.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  a)  At the jobsite, along the front porch of the building, on or about 9/8/15:  Employees were using a ladder jack scaffold to access a section of the roof and to transfer roofing material to the roof where the working platform was placed approximately 3 feet above the surface of the 4:12 pitch porch roof.  The width of the scaffold system would have allowed for only 2 feet or less of surface to prevent a subsequent fall of 10 feet to ground.  Abatement certification must be submitted for this item.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a)  At the jobsite, on the roof, on or about 9/8/15:  Employees were stripping a 4:12 section of roof, with a measured 17.5 foot ground to eave height, without fall protection of any kind.  This condition resulted in an employee falling 17.5 feet from the roof to the ground.      b)  At the jobsite, on the roof, on or about 9/8/15:  Employees were working on a 4:12 section of roof, with a measured 17.5 foot ground to eave height, without adequate fall protection.  Lifelines were observed on the roof without anchor points.      Abatement certification must be submitted for this item.
