29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a. In the JLG 400S aerial lift, fall protection was not provided, exposing employees to an 8 foot fall hazard, on or about 9/11/15. D3 Drywall Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.453(b)(2)(v), which was contained in OSHA inspection number 584718, citation number 1, item number 2 and was affirmed as a final order on 10/26/12, with respect to a workplace located at 2901 East Fowler Ave, Tampa, FL, 33612.
