29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 09/10/15, on the roof of a house at 1074 Parkside Ave., Buffalo, NY: Four employees installing a metal roof on a roof with a pitch of approx. 5 in 12, were exposed to a fall of approx. 20 foot. No fall protection system was being utilized. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 09/10/15, in the driveway of a house at 1074 Parkside Ave., Buffalo, NY: Four employees installing a metal roof accessed the roof by using an extension ladder. The ladder side rails only extended 11.5 inches above the gutter. NO ABATEMENT CERTIFICATION REQUIRED
