29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact.   a) Exterior of Building- Roof Front Porch: Employee without any eye protection was exposed to eye injury while engaged in roofing activities using a pneumatic nail gun , on or about 9/12/15.   b) Exterior of Building - Ground Level - Front Porch: CSHO observed employee without the use of protective helmet exposed to struck by hazards while climbing an aluminum extension ladder below roofing activities, on or about 9/12/15.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     a) Exterior of Building - Roof Front Porch: Employees were exposed to a fall hazard of approximately 10 feet while climbing an aluminum extension ladder that did only extended one foot past the working level being accessed, on or about 9/12/15.
