Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to fall hazards and lateral sliding while performing tasks from a portable ladders:  Jobsite:  The employer did not ensure that the employee, installing snow filter screens over the blast louvers from a portable ladder, was not excessively side loading the ladder and keeping the ladder close to the work to avoid lateral sliding.     On or about August 25, 2015, the above portable ladder laterally slid due to overreaching while the employee was performing the above task, and the employee fell approximately 12 feet onto the Uni-strut that was supporting process piping. The employee sustained serious injuries to the upper arm and face from the incident.
