29 CFR 1910.147(c)(4)(ii)(B): The energy control procedure did not clearly and specifically specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:    (a) In the production area, specific procedures for shutting down, isolating, and securing energy sources were not developed and utilized when servicing and maintaining equipment such as, but not limited to, panel saws and chop saws, exposing the employees to electrical and amputation hazards.

29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources:    (a) In the production area, saw operators responsible for performing saw blade changes on panel and chop saws were not provided with lockout and tagout devices, exposing the employees to an amputation hazard.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices:  (a) In the production area, the employer did not provide training to authorized and affected employees to ensure that the employees understood the purpose and the function of the Company's energy control procedures when performing saw blade changes on panel and chop saws. The employees were exposed to electrical and amputation hazards.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:     (a) In the production department, panel saw operators working on Saw-2 were using compressed air to clean wood chips and debris from the panel saw. The air nozzle being used was not equipped with a diffuser or a safety pressure reducer tip. Air pressure of 105 psi was being released from the nozzle, exposing the employee to a struck-by hazard and air embolism.
