29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:   a)  On or about September 11, 2015 and at times prior to, employees were observed working from a poorly tubular welded frame scaffold not erected by a competent person at a Five Guys Burgers & Fries new building under construction, exposing them to fall hazard.  Mr. Tallman, the Foreman also asked the CSHO what was wrong with the scaffold and he was told that it was not fully planked, and missing guardrails on sides and open ends.
