CFR 1926.501(b) (13):  Each employee engaged residential construction activities 6 feet  (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems;      (a)        Hurst Roofing & Construction, 555 Garfield Dr., grand Junction, CO 81504: On and before September 4, 2015, the employer did not ensure two employees working on the roof of a two story multi-family home were protected from fall hazards through the use of a fall protection system.  Employees were observed performing roofing operations (i.e. shingling) without the use of fall protection.  This condition exposed employees to fall hazards of approximately 15 feet.       Abatement Note:  Abatement certification and documentation is required for this item (See enclosed Sample Abatement-Certification Worksheet).    Hurst Roofing & Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 475479, citation number 1, item number 1 and was affirmed as a final order on July 20th, 2012, with respect to a workplace located at 754 Horizon Drive, Grand Junction, CO 81506.    Hurst Roofing & Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 463958, citation number 1, item number 1 and was affirmed as a final order on July 26th, 2012, with respect to a workplace located at 754 Horizon Dr., Grand Junction, CO 81506.    Hurst Roofing & Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 942839, citation number 1, item number 1 and was affirmed as a final order on December 11th, 2013, with respect to a workplace located at 1453 4th Ave., Grand Junction, CO 81051.
