29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a)  Along the Northeast Side of the Building - On or about September 9, 2015, an employee, putting the Azek trim on the cupola, was not protected from a fall hazard of 16.5 feet from a steeply sloped roof by the use of a guardrail system, safety net system, personal fall arrest system, or alternative fall protection.    No abatement certification or documentation required.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a)  Along the Northeast Side of the Building - On or about September 9, 2015, an employee, putting the Azek trim on the cupola, was exposed to a fall hazard of 16.5 feet because the ladder used to access the roof did not extend 3 feet above the eave.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
