29 CFR 1910.1030(c)(1)(i): The employer having employee(s) with occupational exposure to blood borne pathogens did not establish a written Exposure Control Plan designed to eliminate or minimize employee exposure:     (a) Dental Office -  An Exposure Control Plan had not been provided for the dental assistant or other employees who were at risks of being exposed to bodily fluids (blood and saliva, handling of blood contaminated instruments and equipment) while performing dental operations, on or about September 14, 2015.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1910.1030(c)(2)(i): The employer having employees with occupational exposure to bloods borne pathogens did not prepare an exposure determination to include the elements as listed in 29 CFR 1910.1030(c)(2(i)(A-C):   (a) Dental Office - A dental assistant, dentist and other employees who were occupationally exposed to patients bodily fluids and the handling of blood contaminated instruments and equipment had not been identified in an exposure determination, on or about September 14, 2015.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  (a) Dental Office - Employees conducting dental instrument cleaning operations were exposed to chemicals in using products, including but not limited to Cleanlets, containing 25% of the hazardous ingredient citric acid, on or about September 14, 2015.
