29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: (a) Jobsite: On or about September 14, 2015, an employee was using a three way and the grounding pin had been pulled from it. The employee was using the three way with a double insulated Hilti screw gun.
