Lexington Concrete & Supply, Inc. - Lexington, Ohio: On or about September 2, 2015, the employer did not anchor the 8-inch bench grinder to prevent it from moving during use. The grinder was mounted to a portable pedestal which exposed employees to hazards of the grinder moving/falling.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: Lexington Concrete & Supply, Inc. - Lexington, Ohio: On or about September 2, 2015, the employer failed to ensure that the work rest for the 8-inch bench grinder was adjusted to no more than one-eighth inch from the abrasive wheel. Employees that operated the grinder, which was located in the mechanic shop area, were exposed to injuries from potential jammed work and/or breakage of the wheel.
