29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: (a) During installation of a solar panel system -- On or about 9/15/15, an employee using a self-retracting lanyard with the lanyard connected to a side "D" ring on his harness and extended approximately 18' from the anchorage point was exposed to a swing fall hazard and/or 10' fall with ground contact.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) During installation of a solar panel system -- On or prior to 9/15/15, employees using self-retracting lanyards (SRL) and vertical life lines (VLL) for personal fall arrest were not trained in the recognition and avoidance of swing fall hazards and potential for free fall in excess of 6'. On 9/15/15, an employee using a self-retracting lanyard with the lanyard connected to a side "D" ring on his harness and extended approximately 18' from the anchorage point was exposed to a swing fall hazard and/or 10' fall with ground contact.
