Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) Pack Rat Storage Solutions, LLC on or about September 15, 2015, at a site located on 220 W. Main Street, Wautoma, WI: The employer failed to ensure that employees were provided fall protection while installing roof sheathing on a roof section of the building located on the north east side of the building being remolded. Employees were exposed to falls greater than 19 feet to the ground level. b) Pack Rat Storage Solutions, LLC on or about September 15, 2015, at a site located on 220 W. Main Street, Wautoma, WI: The employer failed to ensure that employees were provided fall protection when exposed to falls greater than 13 feet to the ground while installing wood beam rafters on the south east side of the building.

Main Street, Wautoma, WI: The employer had not provided daily inspections of the trench conducted by a competent person as condition of the trench excavation changed while hand digging the trench. The need for adequate and effective cave-in protection, method of egress, and spoil pile not kept two feet back, was not recognized and addressed while employees were working in the trench.
