29 CFR 1926.150(a)(1): The employer did not develop and maintain a fire protection program to be followed throughout all phases of construction and demolition work, and/or did not provide for the firefighting equiment as specified in this subpart:  a) Roof level: Employees were exposed to fire engulfment hazards while they were using a propane torch to perform roofing work without a fire protection program and without having access to fire extinguishers at all times, on or about 9/15/2015.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) Roof level: Employees were exposed to fall hazards in excess of 23 feet while performing roofing work without the use of guard rails, safety nets, personal fall arrest system, warning line with safety monitor or other feasible means of fall protection. On 9/15/2015, an employee fell approximately 23'-5" and suffered fatal injuries as a result.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:  a) Roof level: Employees were exposed to fall hazards in excess of 23 feet while working on a roof without training on the recognition and avoidance of unsafe conditions or to control or eliminate any fall hazard. On 9/15/2015, an employee fell approximately 23'-5" and suffered fatal injuries as a result .
