(a) Charleston: An employee was working on an 8:12 pitched residential roof of varying heights, with a maximum height of approximately 25 feet from eave to ground without the use of fall protection, as determined on or about September 15, 2015 and times prior.    (b) Charleston: An employee was working on a garage roof that measured approximately 8 feet 6 inches eave to ground on the front side and 11 feet eave to ground on the back side without the use of fall protection, as determined on or about September 15, 2015 and times prior.         ABATEMENT CERTIFICAITON AND DOCUMENTATION REQUIRED FOR THIS ITEM.
