29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: a) Job site, 185 Locust Street, Emsworth, PA 15202: On or about September 14, 2015, during roofing work, the employer did not ensure that employees used appropriate fall protection systems/devices while working on the main house roof on top of the home, where exposed to fall hazards of up to approximately 20 feet. b) Job site, 185 Locust Street, Emsworth, PA 15202: On or about September 14, 2015, during roofing work, the employer did not ensure that employees used appropriate fall protection systems/devices while working on the lower porch roofs of the home, where exposed to fall hazards of up to approximately 10 feet.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: a) Job site, 185 Locust Street, Emsworth, PA 15202: On or about September 14, 2015, during roofing work, the employer did not ensure that a ladder, used to gain access to/egress from the main house roof on the top of the home, was extended 3 feet above the landing surface or that it was tied off and a grab bar provided, exposing an employee to fall hazards of up to approximately 20 feet.
