29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: The employer does not protect employees from falling when working 6 feet or more above a lower level. This violation was observed on or about September 16, 2015 at the job site where employees were exposed to a 12.5 foot fall hazard when installing roof fascia without a fall protection system. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from falling when exposed to falls 6 feet or more above a lower level.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed. The employer does not ensure that ladders are used for the purpose for which they were designed. This violation was observed on or about September 16, 2015 at the job site where employees were exposed to a 8 foot fall hazard when using a step ladder to access the roof for which it was not designed. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that ladders are used for purposes they were designed.
