29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, caught in between, flying chips and sparks: a) Screen printing machine production area: An interlock on the hood of the MPM Corporation Ultraprint 2000 Series screen printer model 2030 serial number 2047 was inoperable allowing the hood to be open during machine operation exposing employees to caught in hazards, on or about 9/16/15. b) MV II chip shooting machine production area: A sliding feeder tray on a Panasonic Panasert MvII chip shooting machine was not guarded exposing employees to a struck-by hazards, on or about 9/16/15. c) MV II chip shooting machine production area: A guard was not in place on the front of the machine where the operator passed and stood during setup exposing the operator to caught in and crushing hazards, on or about 9/16/15.
