29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a- Upper porch South side of building - 2200 Gianna Drive, Glassboro NJ: A company employee, not utilizing head protection,  was exposed to struck by hazards/head injuries when working beneath employees who were using hand tools, hoses, loose roof shingles, etc.  Condition observed on or about 09/12/15.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a- Entire Roof - 2200 Gianna Drive, Glassboro NJ: Three company employees were exposed to fall hazards, in excess of 29 feet to the ground surface below, when performing roof shingle installation and other related activities when either not utilizing any fall protection or when wearing full body harnesses with no lanyards attached.  These employees were working from a roof with a pitch of approximately 7 on 12 at the time of the exposure.  Condition noted on or about 09/12/15.    b- Upper porch South side of building - 2200 Gianna Drive, Glassboro NJ: An employee, who was not wearing a PFAS nor supplied with alternate fall protection, was exposed to fall hazards in excess of 20 feet to the ground surface below when performing clean up and other related activities while at unprotected edges of a porch.   Condition noted on or about 09/12/15.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii)    a- Entire Roof - 2200 Gianna Drive, Glassboro NJ: Three company employees were exposed to fall hazards, in excess of 29 feet to the ground surface below, when performing roof shingle installation and other related activities when either not utilizing any fall protection or when wearing full body harnesses with no lanyards attached.
