29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows: a. Located at 7330 Groveport Road in Groveport, Ohio: On or about September 16, 2015, employees were caulking exterior window frames on the north wall of the recreation center from a Safeway Systems scaffold. Two scaffold platforms were not fully planked, exposing employees to an approximately thirteen foot fall hazard from the upper platform and approximately eight foot fall hazard from the lower platform.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as used as a means of access. a. Located at 7330 Groveport Road in Groveport, Ohio: On or about September 16, 2015, employees were performing caulking work from a Safeway Systems scaffold without an adequate means of access to the working platforms. The first rung of the integrated scaffold access ladder was positioned at forty-four inches from the ground and the scaffold frame access points did not directly access the working platforms, exposing employees to a maximum fall hazard approximately thirteen foot. .

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a. Located at 7330 Groveport Road in Groveport, Ohio: On or about September 16, 2015, employees were caulking exterior window frames on the north wall of the recreation center from a Safeway Systems scaffold which did not have adequate guardrails, to include both a mid-rail and top-rail on all open sides and ends of the upper working platform, thereby exposing employees to an approximately thirteen foot fall hazard.
