29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  (a)  Roof Section Over Front Porch:  Employees were exposed to a fall of 16 feet 2 inches to the ground below while installing siding on a residence from a ladder jack scaffold.  The employees were not utilizing any form of fall protection while on the ladder jack scaffold, as determined on September 17, 2015.  Abatement Certification Required for this Item

29 CFR 1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area:  (a)  Roof Section Over Front Porch:  Employees were exposed to a fall of 16 feet 2 inches to the ground below while installing siding on a residence from a ladder jack scaffold.  The employer has not provided the employees with training on recognition of fall hazards while working on scaffolds, as determined on September 17, 2015.  Abatement Certification Required for this Item.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  (a)  Roof Section Over Front Porch:  Employees was exposed to a fall of 12 feet 2 inches to the ground below while installing siding on a residence from the roof of a residence under construction.  Employees were not protected from falling from the roof to the ground below by a guardrail system or personal fall arrest system, as observed on September 17, 2015.  Abatement Certification Required for this Item.
