On or about September 16, 2015, on the front of the building, employees installing foam insulation from fabricated frame scaffolds with the space between the scaffold planking and uprights exceeding one inch wide, exposing employees to a fall hazard.

29 CFR 1926.451(b)(7): On scaffolds where platforms were overlapped to create a long platform, the overlap did not occur only over supports, and/or was less than 12 inches (30 cm) when the platforms were not nailed together or otherwise restrained to prevent movement. On or about September 16, 2015, on the front of the building, an employee working from fabricated frame scaffolding was exposed to a fall hazard exceeding 10 feet while installing foam insulation without platforms overlapping at least 12 inches or nailed down to prevent movement.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects. For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall. On or about September 16, 2015, on the front side of the building, employees working from fabricated frame scaffolding were exposed to struck-by hazards while installing foam insulation without a means of protection from falling hand tools, debris, or other small objects.
