29 CFR 1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting:  Terminal A and Terminal C, Newark Liberty International Airport, Newark, NJ  a) Employees who perform duties including assisting passengers in wheelchairs, and who expedite unclaimed baggage items from the carousel to American Airlines Carousel 8, have not been trained in the general principles of fire extinguisher use or the hazards involved with incipient stage fire fighting.  b) Employees who perform duties including cleaning passenger plane interiors and dispatching cleaning crews have not been trained in the general principles of fire extinguisher use or the hazards involved with incipient stage fire fighting. 29 CFR 1910.1030(c)(1)(i): The employer having employee(s) with occupational exposure did not establish a written Exposure Control Plan designed to eliminate or minimize employee exposure:  a) Workplace, Newark Liberty International Airport, Newark, NJ  Cabin cleaners perform housekeeping duties inside commercial airlines  including lifting airline seat cushions, and removing all materials from airline seat back pockets.  During such operations workers are exposed to the hazard of needlesticks from medical syringes used and left by passengers.    The employer did not develop a written exposure control plan for  Cabin Cleaners exposed to blood and other potentially infectious materials.  Violation occurred on or about 9-14-15   Abatement Note              The exposure control plan shall contain at least the following elements:                1.    The exposure determination required by paragraph (c)(2);         2.    The schedule and method of implementation;            3.    Methods of Compliance;        4.    Hepatitis B Vaccination and Post-Exposure Evaluation and Follow-Up;            5.    Communication of Hazards to Employees;         6.    Recordkeeping for this standard;           7.    The procedure for the evaluation of circumstances surrounding exposure incidents as required by paragraph (f)(3)(i).

29 CFR 1910.1030(g)(2)(ii)(A): The employer did not ensure that training was provided to employees with occupational exposure at the time of initial assignment to tasks where occupational exposure might take place:  a) Workplace, Newark Liberty International Airport, Newark, NJ  Cabin cleaners perform housekeeping duties inside commercial airlines  including lifting airline seat cushions, and removing all materials from airline seat back pockets.  During such operations workers are exposed to the hazard of needlesticks from medical syringes used and left by passengers.  The employer did not train employees  who are occupationally exposed to blood and other potentially infectious materials in bloodborne pathogens standard requirements.   Violation occurred on or about 9-14-15   Abatement Note  The training program shall contain at a minimum the following elements:  An accessible copy of the regulatory text of this standard and an explanation of its contents;  A general explanation of the epidemiology and symptoms of bloodborne diseases;  An explanation of the modes of transmission of bloodborne pathogens;  An explanation of the employer's exposure control plan and the means by which the employee can obtain a copy of the written plan;  An explanation of the appropriate methods for recognizing tasks and other activities that may involve exposure to blood and other potentially infectious materials;  An explanation of the use and limitations of methods that will prevent or reduce exposure including appropriate engineering controls, work practices, and personal protective equipment;  Information on the types, proper use, location, removal, handling, decontamination and disposal of personal protective equipment;  An explanation of the basis for selection of personal protective equipment;  Information on the hepatitis B vaccine, including information on its efficacy, safety, method of administration, the benefits of being vaccinated, and that the vaccine and vaccination will be offered free of charge;  Information on the appropriate actions to take and persons to contact in an emergency involving blood or other potentially infectious materials;  An explanation of the procedure to follow if an exposure incident occurs, including the method of of reporting the incident and the medical follow-up that will be made available;  Information on the post-exposure evaluation and follow-up that the employer is required to provide for the employee following an exposure incident;  An explanation of the signs and labels and/or color coding required by paragraph (g)(1);   An opportunity for interactive questions and answers with the person conducting the training session.  The person conducting the training shall be knowledgeable in the subject matter covered by the elements contained in the training program as it relates to the workplace that the training will address.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment:  a) Workplace, Newark Liberty International Airport, Newark, NJ  Employees engaged in expediter operations perform tasks including but not limited to transferring passenger luggage between connecting flights and can encounter cuts/abrasions according to the employer's existing hazard assessment.  Such employees are also instructed by managers to to clean up liquid spills or leaks from baggage when such events occur.  The employer's hazard assessment did not include a requirement for use of hand protection for such hazards.   Violation occurred on or about 9-14-15
