29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about 9/3/2015, the employer failed to provide adequate guarding to the Pellet machine, in that the guard installed had been designed to fit multiple different size production pieces without adjustment. The opening was about 10 inches in height, 1.5 inches wide on the right side, 12 inches in height, and 1.5 inches in width on the left side. Near the bottom of the right side, the opening of the guard was about 3.5 inches.
