29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous. a. At the temporary panel located outside of the rear of the house where employees plugged in an extension cord, the ground prong was missing from the extension cord, thereby exposing employees to an electrical contact hazard. b. In the rear of the house where employees plugged a 2 foot 3-way extension cord into an extension cord, the ground prong was missing from the 3-way extension cord, thereby exposing employees to an electrical contact hazard. c. In the rear of the house where employees plugged in an extension cord into a 2 foot 3-way extension cord, the ground prong was missing from the extension cord, thereby exposing employees to an electrical contact hazard.

29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. a. In the first floor hallway where employees used a Milwaukee hole hog to drill holes for the installation of the plumbing, the strain relief was pulled away from the outer insulation of the cord at the tool end, thereby exposing the employees to an electrical contact hazard.
