OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. a). On or about September 15, 2015 employees were operating Crown Narrow Reach Lift trucks, Series RR5200 electric lifts and were exposed to hazards that would result in death or serious permanent physical injuries because; The operators Field of vision or view was blocked at intersections and corners because the operators position inside the lift prevents a full 180 degree of vision while traveling. There are no posted speed limits for travel within the warehouse. There are no stop signs at intersections or ends of the aisles. There is no procedure or instructions to encourage the operator to keep all body parts inside the machine while traveling. There are no blind spot mirrors mounted on the lift trucks to aid and enhance operators vision while traveling through the warehouse.
