29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: At this worksite, an employee installed a sewer line inside a trench that measured five feet and eleven inches deep at its deepest point and ten feet 5 inches long and seven feet eight inches wide. A protective system was not used exposing the employee to hazards associated with a cave-in.
