29 CFR 1926.405(b)(1): Conductors entering boxes, cabinets, or fittings. Conductors entering boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed. Unused openings in cabinets, boxes, and fittings shall also be effectively closed. a. On the first floor of the Bistro Area B where the temporary electric was set up, there was one unused opening which measured 3 inches in diameter on the main panel connected to the generator thereby exposing employees to an electrical hazard. b. On the first floor of the Bistro Area B where the temporary electric was set up, there was one unused opening which measured 3/4 inches in diameter on the main panel connected to the generator thereby exposing employees to an electrical hazard.

29 CFR 1926.405(g)(2)(iii): Splices. Flexible cords shall be used only in continuous lengths without splice or tap. Hard service flexible cords No. 12 or larger may be repaired if spliced so that the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced. a. On the first floor Area A-1, the electrical contractor spliced two pieces of romex together just below the receptacle and covered partially with electrical tape thereby exposing employees to an electrical hazard.
