29 CFR 1910.27(b)(1)(ii): The distance between rungs exceeded 12 inches and were not uniform throughout the ladder: Utility Room, Roof access ladder, 150 New Athol Road, Athol, MA. On or about September 16, 2015 employees were required to use a fixed steel ladder to access roof top mechanical systems which had rungs that were both not uniform and exceeded 12 inches between rungs.
