29 CFR 1926.304(h)(1): Each circular crosscut table saw was not guarded by a hood which completely enclosed that portion of the saw blade above the table and that portion of the saw above the material being cut:  (a) Jobsite - Rear of structure:  On or about 9/18/2015, employees were not protected from injuries when working with a table saw that had the guard removed, while cutting wood.

29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee.  (a) Jobsite - Roof:  On or about 9/18/2015, two employees attached to the same anchor point were not protected from falling when using a PFAS system which did not have a fully secured anchor point capable of supporting at least 5000 pounds per employee.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a)  Jobsite - Roof:  On or about 9/18/2015, an employee wearing a body belt was not adequately protected from falling while working on a roof approximately 29 feet above a lower level.  H&R Builders and Renovation, LLC was previously cited for a violation of this occupational safety and health standard, or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 772401, citation number 001, item number 03(b) and was affirmed as a final order on 5/31/2013, with respect to a workplace located at 350 Pocasett Avenue, Providence RI, 02909.

H&R Builders and Renovations, LLC was previously cited for a violation of this occupational safety and health standard, or its equivalent standard 1926.503(a)(1), which was contained in OSHA inspection number 772401, citation number 001, item number 04(a) and was affirmed as a final order on 5/31/2013, with respect to a workplace located at 350 Pocasett Avenue, Providence, RI 02909.
