29 CFR 1926.351(b)(4): Welding cable(s) in use were in need of repair.  a). At the Worksite - On or about and times prior to September 16, 2015, a MultiQuip DC Welder/AC Generator (model: DLW-330X2) was used with welding cables that were damaged in multiple locations, thus exposing employees to electrical hazards.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift. Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated.  a). At the Worksite - On or about September 16, 2015, personal fall protection equipment was not worn by employees using a Genie Articulating Boom Lift (model: Z-45/25J), thus exposing employees to a fall hazard of approximately 21.4 feet on earthen ground.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a). At the Worksite - On or about September 16, 2015, an employee was working on a catwalk that did not have a fall protection system in place, thus exposing the employee to a fall hazard of approximately 21.4 feet onto earthen ground.
