29 CFR 1910.23(b)(1): Every wall opening from which there is a drop of more than 4 feet shall be guarded by one of the following: 29 CFR 1910.23(b)(1)(i): Rail, roller, picket fence, half door, or equivalent barrier. Where there is exposure below to falling materials, a removable toe board or the equivalent shall also be provided. When the opening is not in use for handling materials, the guard shall be kept in position regardless of a door on the opening. In addition, a grab handle shall be provided on each side of the opening with its center approximately 4 feet above floor level and of standard strength and mounting. a. On or about September 10, 2015, located at the Huffman Dining Hall at Denison University, employees operated a Perkins Manufacturing Company Industrial Lifter to dump trash into garbage dumpsters and took smoke breaks near an unguarded retaining wall, thereby exposing the employees to a fall hazard of 4 feet, 2 inches.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws. a. On or about September 10, 2015, located at the Huffman Dining Hall at Denison University, employees operated a Perkins Manufacturing Company Industrial lift in order to dump trash/compost into garbage dumpsters. Below the operator's controls, there was no strain relief provided for the flexible cords that were used in part to supply a 3-phase 208 VAC electric motor, thereby exposing the employees to an electrical shock hazard.
